Passports: Change of Legal Name
Effective September 26, 2005, as an additional security enhancement, Passport Services will no longer amend valid passports. Instead, customers requesting name changes will apply for replacement passports. You may submit your application by mail to:

There are 2 separate types of applications with different specifications.
- A. If you apply to have your name changed within one year of your current passport’s issuance date (i.e. you current passport is less than one year old) please click here
- B. If you apply to have your name changed one year or later after your current passport’s issuance date (i.e. your current passport is more than one year old) please click here
