Forms

There are a number of forms required for visa applications depending on the type of visa, age, gender, and nationality of the applicant. All visa applicants must complete DS-156. Males age 16-45 must fill out the DS-157 and students (F and J applicants and their dependents) must submit the DS-158. PLEASE NOTE THAT ALL FOREIGN NATIONALS (NON-FINNS) ARE TO COMPLETE THE DS-157 AND DS-158 AS PART OF THEIR APPLICATION. Below is a list of the forms and who needs to submit them with their application.

PLEASE ALSO ANSWER TO QUESTION NUMBER 6 WITH INFORMATION ABOUT YOUR CHILDREN AND PARENTS AND SIBLINGS. EVEN IF THEY DO NOT LIVE IN FINLAND AND DO NOT TRAVEL WITH YOU TO THE US.
